Job Title: Case Manager

Mission: To work as a Case Manager with a team of clinicians, client services staff, interns and team members who collectively deliver the highest quality care using a client-centered, strength-based case management model, ensuring holistic care and a strong organizational culture.


  • Conducts accurate Case Management Needs Assessment and consistently re-screens for additional needs.
  • Creates individualized Case Management Care Plans that include multidisciplinary team input as well as involvement of the patient, referent, family, and all other appropriate collateral resources.
  • Provides professional case management services that facilitate coordination, communication, and collaboration with clients, providers, ancillary services, and others in order to achieve goals and maximize positive client outcomes based upon individual assessments of clients’ needs.
  • Maintains at least a 95% completion rate for Case Management documentation.
  • Facilitates consistent communication with family members/significant others.
  • Maintains very high client and family satisfaction rates, as evidenced by a strong net promoter score and/or client experience surveys.
  • Researches and connects with potentially beneficial resources within the community, both in the public and private sector, aimed at meeting all the specific needs of the clientele.
  • Compile and maintain binders and files full of up-to-date referral and resources information.
  • Produces and ensures high quality and accurate case management documentation along with consistent, on-time submission of all required documentation.
  • Regularly attends and thoughtfully participates in staffing meetings, providing clear case conceptualizations, and offering key information / input as needed.
  • Manages up, providing concrete potential solutions if able, when bringing issues and questions to supervisors.
  • Has a keen of understanding of regulations and ethics pertaining to the state license(s) and national accreditation(s) held by licensing board.
  • Maintains a working knowledge of the policies and procedures of the facility, holding yourself and other staff accountable to the expectations of the organization as a whole.
  • Consistently responsive to all emails and phone calls both internally and externally.

Core capabilities and experience desired for this specific role

  • 1 or more years providing chemical dependency and/or mental health case management preferred
  • Familiarity with Fort Collins community resources
  • Minimum two years freedom from chemical use problems
  • Excellent leadership and interpersonal skills
  • Strong and clear communication skills
  • Strong initiative and very resourceful in creating and finding ways to get things done
  • Solid team-player
  • Punctual with meetings and project timelines
  • Familiar with EHR software, Microsoft suite and basic computer skills
